Minoxidil topique 5 % une fois par jour + spironolactone orale 80 à 100 mg par jour

Chez des femmes non ménopausées de 18 à 45 ans atteintes d'alopécie androgénétique, un seul essai de fiabilité moyenne a évalué la spironolactone orale 80 à 100 mg ajoutée au minoxidil 5 % une fois par jour (Liang 2022). Le nombre de cheveux passe de 99,14 à 115,89 par cm² en 24 semaines, significativement plus qu'avec le minoxidil seul ou qu'avec le microneedling électrique associé au minoxidil, et 86,49 % des femmes sont améliorées sur photos jugées à l'aveugle ; l'épaisseur du cheveu, elle, ne diffère pas significativement. Côté tolérance, 15 femmes sur 37 ont eu des règles irrégulières, 8 sur 37 des démangeaisons et 1 sur 37 un potassium trop élevé dans le sang.
