Le minoxidil sublingual 0,45 mg par jour n'a été évalué que dans deux petits essais contre placebo, de fiabilité limitée. Chez des hommes et des femmes de 30 à 65 ans atteints d'alopécie androgénétique, le gain atteint +9 cheveux épais par cm² au vertex et +4 dans la zone frontale en 24 semaines, et 55 % des patients sont améliorés sur photos jugées à l'aveugle au vertex, 45 % dans la zone frontale (Bokhari 2022). Chez l'homme, l'épaisseur du cheveu baisse de 2 µm, autant que sous placebo (Sinclair 2025). Ce sont des indices, à confirmer.

Results After 24 weeks, fibre diameter decreased by 2 µm (SD 1) in men receiving placebo. The decrease was most marked in intermediate diameter fibres with a baseline diameter of 40–60 µm.
