Le minoxidil sublingual 4,05 mg une fois par jour n'a été suivi que dans deux phases d'extension ouvertes, sans groupe de comparaison et de fiabilité limitée. Chez 6 participants passés à cette dose plus forte, 67 % (4 sur 6) sont améliorés sur photos jugées à l'aveugle en 24 semaines, au vertex comme dans la zone frontale, avec un gain de +88 cheveux épais par cm² au vertex et de +38 dans la zone frontale (Bokhari 2022). Chez des hommes atteints d'alopécie androgénétique qui avaient d'abord reçu le placebo, le cheveu s'épaissit de +6 µm en 24 semaines (Sinclair 2025). Ce sont des indices préliminaires, à confirmer.

Results After 24 weeks, fibre diameter decreased by 2 µm (SD 1) in men receiving placebo. The decrease was most marked in intermediate diameter fibres with a baseline diameter of 40–60 µm.
